This website does not collect health information

Booking, your health intake, consent and payment are all completed through Jane, a HIPAA-compliant practice management platform that we use to schedule visits and keep clinical records. Health information you give during booking goes to Jane, not to this website, and is protected under our agreement with them.

Please do not send medical information through this website's forms or by email. Your nurse reviews your intake privately before your visit.

What this website collects

Only what you choose to send us, and basic page statistics:

• Your name, phone number and email, if you request a group or event quote, and whether you agreed to receive text messages
• Your name, city and comments, if you leave a public review
• Anonymous page-view statistics on public pages: which page was viewed, never anything you type

How we use it

We use contact details only to reply to your request. Reviews are published after staff review, and may be declined.

Health information held in Jane is used to provide the care you book, and may be shared with our physician-owned professional corporation and supervising physicians for care coordination. It is not shared more widely than that without your authorization, except where the law requires it.

We do not sell your information

We do not sell personal or health information. We do not trade it, and we do not provide it to advertisers or data brokers.

Text messages

If you agree to receive text messages from us, for example when you request a group or event quote, we use your mobile number only to text you about that request and your visit. Agreeing is optional. Message frequency varies, and message and data rates may apply. Reply STOP to any message to stop, or HELP for help.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text-messaging opt-in data and consent are not shared with any third party, except the service provider that delivers our messages.

Your rights over your medical records

For the records created when you receive care, you have the right to:

• Access your records and get a copy
• Request corrections to information you believe is wrong
• Request restrictions on how your information is used or shared
• Request an accounting of disclosures: a list of where your information has gone
